There are several
ways of open accounts with us. You may come into the store, you may call
on the telephone, or you may try this 21st Century way by filling out this
on-line form for application.
If you are a
Previous Customer in the past at one of our store,
follow this link to fill out the
appropriate form for a previous customer.
Previous customers will fill out a form that will try to find your name
in a data base of customers. If we are able to do that, then the
application process will be faster.
If you are a
New Customer applying for the first time,
follow this link to fill out the appropriate form for a
new customer. When filling out
this form as a new customer, please try to fill the form out as completely as
possible. There are some questions that MUST be answered, and if they are
not, then application will not be complete; this may result in the application
process being done in the pharmacy or over the telephone.
